Bequest Language
Planning for the future and creating the legacy you wish to leave is one of the most effective ways to ensure a lasting impact. Making a gift in your will or living trust, known as a bequest, can guarantee that your legacy endures. Your gift may be a specific dollar amount, a percentage of your estate or the remainder after all other provisions are specified.

For an unrestricted gift that allows Cook Children's Health Foundation to determine how to use the funds based on the most pressing needs, use the following language for your will and/or trust:
"I, [name], of [city, state, zip], give, devise and bequeath to W.I. Cook Foundation, Inc. dba Cook Children’s Health Foundation, a non-profit corporation currently located at 801 7th Avenue, Fort Worth, TX 76104-2796, _________ [written amount or percentage of the estate or description of property] for its unrestricted charitable use and purpose."
To designate your provision for a particular purpose:
"I, [name], of [city, state, zip], give, devise and bequeath to W.I. Cook Foundation, Inc. dba Cook Children’s Health Foundation, a non-profit corporation currently located at 801 7th Avenue, Fort Worth, TX 76104-2796, _________ [written amount or percentage of the estate or description of property] for the purpose of [insert purpose]. If in the judgement of the Board of Cook Children’s Health Foundation, changed circumstances should at some future time render the designated use of this gift no longer appropriate, then the Board shall use the gift to further the objects and purposes of Cook Children’s giving consideration to my special interest as evidenced by the purposes described above."
